Physiotherapy Agreement
This agreement was last amended on 10 September 2019. The objective of this agreement is to meet customer expectations regarding our physiotherapy and related services, equipment deliveries, medical and training services and to ensure that we maintain a high level of service. This agreement describes the parameters of all physiotherapy and related services, equipment deliveries, medico-legal and training services that are mutually understood by the directors. This Agreement does not replace ongoing processes and procedures unless expressly stated. Effective management of physiotherapeutic services is the result of maintaining a consistent level of service. The following sections contain relevant information on the availability of the service and the handling of complaints. CRA asks the parties to the employment contract what their intention was when they concluded the employment agreement. The CRA wants to know how they define their employment relationship and why they define it as such. Sometimes the intention is clear and both parties agree (common intention). Sometimes the parties have differing views on the status of their employment relationship, in which case there is no common intention. This agreement remains valid until it is replaced by a revised agreement approved by the directors.
The Client Services Supervisor (“Document Owner”) is responsible for enabling regular verification of this document. The content of this document may be amended if necessary, provided that the directors have given their mutual agreement and are communicated to all parties concerned. The document holder will include all subsequent revisions and, if necessary, request reciprocal agreements/authorizations. The purpose of this Agreement is to ensure that the appropriate elements and obligations are in place to provide consistent physiotherapy and related services, equipment, medical and training services, assistance and delivery to the customer by PhysioFunction. This agreement constitutes a service level agreement (“SLA” or “agreement”) between PhysioFunction and our customers for the provision of physiotherapy and related services, equipment, forensic and training services. . . .
